Cognitive Assessment use by Speech-Language Pathologists
You are invited to complete this survey which examines the use of formal and informal cognitive assessments by speech-language pathologists. You are an eligible participant because you: (a) hold clinical certification or are in the clinical fellowship stage as a speech-language pathologist, (b) you routinely administer cognitive assessments with your adolescent/adult patients, (c) you routinely work with adolescent and adult individuals with histories of cognitive disorders (e.g., TBI, aphasia, dementia), (d) you speak/read English, (e) you have access to the internet for survey completion, and (f) you are between 18 - 80 years old.
If you participate, you will do the following:
Provide brief demographic background information regarding you and your work setting (e.g., age, gender, employment environment, years of experience, typical patient load, etc.)
Answer questions regarding cognitive assessment use, including but not limited to, types of assessments administered, frequency of assessments given, and preferences for various assessments
Have the opportunity to enter into a drawing for a $50.00 Amazon gift card
This survey is expected to take less than 30-minutes to complete. This survey will close on May 12th, 2017.
Risks and Benefits:
You may feel uncomfortable when answering specific questions on this survey. You may skip questions that you do not feel comfortable answering. There are no direct benefits to participating in this study.
At the end of the survey, you will have the option to be entered into a drawing for a $50.00 Amazon Gift Card. For every 50 participants who complete the survey and choose to be entered into a drawing, one name will be selected. This person will be emailed the gift card. If you enter into the drawing, you have a 1 in 50 chance of receiving the gift card.
The records of this study will be kept private. In any sort of report we might publish, we will exclude any information that will make it possible to identify a subject. Research records will be stored securely and only researchers will have access to these records. All study data will be encrypted. These records will be accessed only by members of the research team.
Voluntary Nature of the Study:
Completion of this survey is voluntary. If you decide to participate, you are free to withdraw at any time. Your decision whether or not to participate will not affect your current or future relations with the University of Minnesota.
